It is vitally important that the young people move on to an appropriate placement and continue to build on the skills they have learned during their time at school.

It is important that the next steps are discussed and explored well in advance of the end of the school programme.

All young people 14-19 have a Transition Plan that is formally updated during their annual reviews. It is a statutory requirement that Connexions / careers advisers are fully involved in these discussions as they process applications and other required documentation. They will be able to provide information about the full range of options. It is important that all options are considered to ensure informed decisions are made. The placement is discussed and identified in Year 13 and Year 14 reviews.

An application to any placement college, home etc, will need an application form to be completed and an assessment undertaken. Local provision will have to be first discounted if families are looking for an out of county placement.

Transition activity is planned into the class programmes particularly during the final year of their school education. This activity takes many forms, depending on what has been planned for the next stage.

We are pleased that over the last few years all the young people have moved onto appropriate placements.
