St Elizabeth’s College prides itself on the quality of its transitions planning which is led by a specialist co-ordinator.
Learners move on to a wide range of settings. For instance, some are able to move into supported independent living while others need more support which can include 24 hour care. Our case studies, which you can see in the link below, provide some examples.
Formal planning for the future starts from the annual reviews, during April and May of the second year. From the start of the third year, learners have formal and informal sessions with the Transitions Co-ordinator. The learners are fully supported to express their hopes and dreams and these are incorporated into a person centered plan with input from parents and carers. We maintain excellent relationships with all the crucial agencies and providers ensuring that our learners leave with a full package that meets their individual needs.
